The Sample Workplan complements the Employment Supports Transformation Assessment (ESTA) tool by providing a framework for identifying, planning, and implementing change. The Sample Workplan includes all nine domain areas found in the ESTA and offers recommendations and suggestions for action steps and tasks to be accomplished in each domain area. The workplan should be populated and individualized according to the areas of need that are identified when the agency completes the ESTA.

The workplan should become a “living document” that serves as the central focus of the agency’s transformation efforts. Agency Transformation Team meetings should use the workplan to guide the planning process and track progress. Tasks may be added or deleted under each domain as challenges are resolved, solutions are identified, and goals are met.
